Each plan includes a sample client assessment database, nursing priorities, potential nursing diagnoses with "May be related to" and "Possibly evidenced by" statements, desired outcomes/evaluation criteria, interventions, and rationales for each intervention. The interventions are separated into independent and collaborative actions A specific assessment tool for data collection is included at the beginning of each section.
